HYALURONIC ACID: From Facial Aesthetics to Joint Health

Each joint is composed of the following elements:

This inner membrane nourishes the cartilage and produces “joint juice,” the synovial fluid, which reduces friction in joint movement, protecting them from wear and tear. When the pressure on the joint is reduced, the cartilage absorbs the joint fluid like a sponge, and when the pressure increases, the cartilage eliminates this fluid according to the degree of loading. At the same time, synovial fluid isolates the joint surfaces from each other, forming a smooth surface. A healthy joint “lubricates” itself, which is why the frictional force in a healthy joint is 5 times greater than the frictional force on the ice.
The bone ends (epiphyses) are protected by hyaline cartilage with a height of 3-5 mm, which serves as a “protective cushion”: the cartilage absorbs heavy loads and sudden movements. Articular cartilage is not connected to the bloodstream, so it does not receive nutrients from the blood. For cartilage, the source of these substances is the joint fluid. During movement, cartilage cells absorb the necessary nutrients from the joint fluid and release metabolites, which is why joint movement illustrates the true meaning of the word “life.”

The base of the articular cartilage is the matrix (intercellular substance) composed of a network of collagen fibres. Within the matrix are chondrocytes – cells that synthesize proteins for collagen fibers and the structural material for the cartilage base. The matrix is composed of very large molecules (proteoglycans) that can retain a large amount of water, providing elasticity and cushioning against impacts. Additionally, the matrix is rich in glycosaminoglycans, hyaluronic acid, and chondroitin sulfate – structural components of synovial fluid.

At the age of 40, the production of endogenous hyaluronic acid decreases by 50%. At 60 and above, the body produces only 10% of the required amount.
Hyaluronic acid can retain up to six litres of water per gram. Water cannot be compressed, and hyaluronic acid transfers this property to the joints. Hyaluronic acid is responsible for the consistency of the tissue fluid and modifies its viscosity according to mechanical forces: the fluid thins during the increased effort but remains viscous enough to prevent it from disappearing from the joints as water.
